If they were not covered when they were on assignments, note this and we can document they weren t covered. We will need a COI showing evidence of coverage for the time at Sanford. This needs to list their name on the document. Copy of training/diploma Copy of board certification Evidence of clinical activity-40 encounters in the last 12 months Notarized ID Employee Heath clearance which includes TB titer and urine drug screen. If coming between December and May, need to have evidence of flu vaccine or exemption form completed.*MUST note in submission if clinician has ever worked for Sanford Health. If clinician previously worked with Sanford, they must have an end date 6 months prior to their submission date.* Cancellation Policy:Client may cancel up to 12 hours every 13 weeks
