Saturday, Sunday and Monday 8am-6pm. Description: Shift: Monday to Friday 8 am to 5 pm with flexibility based on patient needs. Scheduled for 1 weekend a month (followed by 2 days off: 1 day prior to Saturday and 1 day after Sunday).Call: Some call may be scheduled, but only on an emergency basisRequirements:-Must have OASIS (Outcome and Assessment Information Set) Experience-Home Health and Hospice Experience required-Experience traveling from patient to patient for home visits-Must be able to manage and adjust to changes in schedule-Must have drivers license in good standing-BLS required-EMR system used is HealthWyse (Careficient)-Outpatient care-Facility does their own Background Check as wellCome join our team today!!
