INFOPATIENT DIAGNOSES: - CHF- A-FIB- VALVULAR DISEASE - PACEMAKERSSPECIAL PROCEDURES/UNIT DETAILS - DOBUTAMINE STRESS TESTING SKILLS - IV INSERTION (PREFERRED)- PHLEBOTOMY (PREFERRED)Case manager to work with 2 providers in the clinic to be available to complete patient intakes, IV insertions, and phlebotomy as needed. Case manager also will manage provider's schedule and assist with scheduling patients as needed. Triage patient phone calls. Propose medication refills and orders. Scheduling surgeries. Managing external referrals.
