Assignment Details The Home Health Physical Therapist plans and administers medically prescribed physical therapy treatments for patients suffering from injuries, or muscle, nerve, joint, and bone diseases, in an effort to restore function, relieve pain, and prevent disability by performing the following duties: providing education and information to patients and caregivers; participating in required discharge planning, and completing all required documentation according to policy; and directing patient participation in selected tasks to restore, reinforce, and enhance performance.Schedule: 8:00 to 5:00 PM M-FWeekend for Ortho cases only! For PT every 4th or 5th weekend Unit InformationMileage reimbursed at IRS rate6 to 7 Weighted visits per day Minimum Requirements 1 year experience required - Home Health experience preferredUnit is open to first time travelers! MatrixCare or Epic experience preferred, not required Required Certifications VA PT State License Required - CAN BE PENDING!!!!
