Day Description: Requisition

Comments and Notes: Rates are tiered based on mileage - measured by direct line not driving distance. Travel (101 miles), Radius (51-100 miles), Local (50 miles or less) RN for Outpatient Dialysis, Joy Dr. Address: 35 Joy Dr., So. Burlington VT 05403. Must be flexible with schedule - will work 8/9/10/12H shifts as needed. Floating as needed to St. Albans & Berlin is required. No call but Saturday requirements.

ESRD: Chronic renal patients. Requirements:  AHA BLS    Prior travel experience preferred  A minimum of 2 years of experience required  A minimum of 1 year of outpatient experience required  EPIC experience from within the past 2 years is required and nonnegotiable Additional Information:  IVS to OFFER Monday to Sunday  No block schedules for any reason    Bi-weekly orientations on Monday Please ensure all employment gaps over 90 days are explained  This facility requires ALL work history information (bed count, EMR, facility type, etc.) to be complete in order to submitDisclaimers:  Public Transportation is NOT accessible in this area, please ensure you have reliable transportation, personal vehicle or rental vehicle are strongly recommended  Vehicles should be equipped with all season or snow tires during the months of November April Float Required: NoCertifications: BLS
