Services are provided based on the ages of the population served; Children through geriatric - 65 years or older and in collaboration with the multidisciplinary care team. Assesses, plans, implements and evaluates care in collaboration with the multidisciplinary team. Demonstrates the knowledge and skill necessary to identify and meet the individualized needs of all patients served. The Physical therapist is under the supervision of the Director of Rehabilitation and Sports Medicine and Rehabilitation Service Manager.QualificationsEducation:

Graduation from an accredited

Physical Therapy programExperience

Two (2) years experience as a Physical Therapist preferred

Recent / previous work experience in outpatient and/or inpatient experience preferredKnowledge/Skills/Abilities:

Ability to follow written and oral directions

Computer skills required

Demonstrates high level of personal and professional accountability and responsibility
