It entails phone triage, very rare does it require a home visit. 1- 2 weeks a month.What is Health Care Connectors productivity requirement for Home Health RNs? - It is a 30 pt per week for full time Nurses. Our point system:- 1 revisit= 1pt ,- SOC= 2pts , - ROC = 2pts- Recert =1.5pt- Oasis discharge =1.5pt
