National Criminal Check, FACIS Level III, National Sex Offender, all counties lived and worked within past 7 years, including for any aliases. SSN Address Trace required.OIG (Office of Inspector General) Completed prior to or within thirty (30) days of start date and at time of extension. Include alias names.SAM Completed prior to or within thirty (30) days of start date and at time of extension. Include alias names.MMR Proof of two (2) MMR vaccinations or positive IGG titers. A decline form will be accepted after results of low or equivocal titers.Varicella Proof of two (2) vaccinations, or positive IGG titer. A decline form will be accepted after results of low or equivocal titers.Hep B Proof of vaccination series, positive surface antibody titer or declination form.TB Documentation of two (2) TB skin tests, one within a year of start date and the other TB test within thirty (30) days of start date. TB skin test should be repeated annually thereafter. TB QuantiFeron completed within one (1) year of start date and annually thereafter would also be acceptable. Chest X-Ray should only be completed for past positive PPD cases as a result of TB exposure. Chest X-Ray result must indicate that the Chest X-Ray was completed to rule out positive TB results. The Chest X-Ray results are good for 5 years. If providing Chest X-Ray, past positive TB result and TB questionnaire is required at time of placement. An 80% or passing score is required.Joint Commission - Core I, II, III Modules Completed within one (1) year of start date; required annually. An 80% or passing score is required.Valid state issued Drivers License or ID card State issued photo ID or state issued Drivers License required to be current at time of start. Photo ID must be submitted to clients for identification verification.BLS BLS certification (AHA) is required for all patient-facing HCPs. Any certification required must be kept current throughout assignment.E-Verify Upload E-Verify resultsPre employment physical Completed within one (1) year of start date; required annually. Must be signed by MD, DO, DC, NP, or PA.Healthcare Provider Timekeeping Instructions Agency to download this document and provide to all HCP's prior to their start date.
